Position Purpose: to promote the Church as a place of health, faith and healing

Responsibilities: As a member of the ministerial team of Yorkminster Park Baptist Church, the Parish Nurse is
responsible for the assessment, planning, implementation and evaluation of a faith-based nursing practice focused on health promotion and the integration of faith and health. The Parish Nurse provides an intentional health ministry across the life span integrating concepts of health and illness within the context of a faith community

Specific Responsibilities

Health Counselor:

• Provides individual and family counseling regarding issues related to health and the response to change and loss. 

• Provides home visits, visits to hospitals and long-term care homes in consultation with the Minister of Pastoral Care.

Referral Agent:

• Links individuals and families to appropriate community resources and evaluates the outcome of the linkage/referral

• Assists individuals to navigate the complex world of health and social services

Health Advocate:

• Assists individuals to represent their needs as appropriate

• Appropriately involves advocacy on a communal level such as participating in the development of programs for vulnerable populations

Health Educator:

• Designs, delivers and evaluates appropriate health promotion educational sessions/activities based on assessed need

Support Groups:

• Arranges or develops support groups, such as Bereavement and Divorce Care, in partnership with ministerial staff, church groups, community agencies; facilitate access to support groups for individuals.

Integrates Faith and Health:

• Develops programs and approaches that consider the nature of health, illness, suffering or loss in the context of the Christian faith. The Parish Nurse integrates health seeking and faith seeking.

First Aid and Emergency Response:

• Is responsible for the oversight of first aid kits, first aid training and training in use of defibrillator for designated staff and volunteers.

Community Partnerships:

• Maintains and develops community partnerships (such as The Anne Johnson Health Station and Sunnybrook Community Committee)

• Attends Sunday morning worship service on a regular basis. These hours are part of the scheduled hours of the position; currently 21 hours per week.

• Meets regularly with members of the Ministry team and with the Health and Wellness Committee

• Maintains client records in accordance with documentation standards of the College of Nurses of Ontario

• Links with the Parish Nurse Interest Group of the RNAO and attend conferences wherever possible

Qualifications, Skills and Abilities:

• Registered Nurse in good standing with the College of Nurses of Ontario, with a minimum of 5 years nursing experience

• Community nursing or teaching experience an asset

• Professional liability insurance is required

• Baccalaureate preferred

• Certificate or courses in Parish Nursing is required

• Well grounded in personal faith and spiritually mature

Demonstrated
  - ability to walk with those who suffer 
  - ability to work effectively with diverse groups of people
  - ability to work independently
  - strong written and verbal communication skills
  - teaching ability

• Experienced in working effectively with a volunteer board or committee

• Is knowledgeable about community resources

• Experienced in a liaison role and linking individuals to resources

Accountability: The Parish Nurse is a member of the ministry team, and reports to the Minister of Pastoral Care, the Health and Wellness Committee and the Pastoral Care Committee

Hours of Practice: 21 hours per week. This includes attending a Sunday worship service.

Evaluation:

• The Parish Nurse develops an annual program plan for approval

• Meets regularly with the ministry team and the Health and Wellness Committee to review activities and plans.

• Completes self-evaluation based on reflective practice guidelines of the College of Nurses of Ontario

• Receives a written annual review completed by the Minister of Pastoral Care and the Health and Wellness Committee
